After you complete this form, it will be reviewed by a Fraternal of The 15 Association. If you are a match for us, you will receive an email with a link to complete the rest of the information required to be put on the Door List for the event. You must click on the link in the email and complete the form in order to be put on the door list.

We welcome guests who are active members of the BDSM community. If you attend a 15 event, although you are not required to play at the party, the expectation is that you are interested in doing so. People interested in voyeurism only are not a good fit for our events.

Please note that these events are not training classes, and no formal instruction is provided.

To request an invitation to our SMBD play party you must be 21 years of age or older and have a Photo ID that states you are male.

Please provide the information below, and be specific for the essay-style questions. Flippant, brief answers will most likely be rejected.  Your email address must be capable of receiving mail, or you will not receive the response to whether your request has been approved or rejected.

ONLY ONE PERSON PER FORM!  If multiple people are requesting entrance to our parties, EACH ONE MUST BE A SEPARATE REQUEST.
